Physical Examination
Students registering for Athletics must have a current physical examination form submitted before the student is authorized to participate in practice or tryouts. Download the physical form by clicking the athletic page of the Milton High School website.
PHYSICAL REQUIREMENTS FOR 2023-24 SCHOOL YEAR
BEFORE APRIL 1, 2022 - NEED PHYSICAL
AFTER APRIL 1, 2022-MARCH 31, 2023- GOOD FOR 1 MORE YEAR
APRIL 1, 2023-CURRENT DATE - GOOD FOR 2 YEARS
